We are hiring a Full-Time ER/Hospitalist ARNP/PA to be a part of our team of experienced professionals in a setting where you will work collaboratively with medical staff to coordinate treatment plans and referrals while meeting and maintaining regulatory requirements set forth by CMS Conditions of Participation.
Job SummaryThe Advanced Practice Provider (APP) provides acute primary health care to hospital inpatients, observation patients and swing bed patients while dually performing duties relative to the management and treatment of acute patient injuries and illness in the Emergency Room setting.
APPs work collaboratively with Medical Staff to coordinate treatment plans and referral while meeting and maintaining regulatory requirements set forth by CMS Conditions of Participation.
GMHC values collaboration and team-oriented partnerships, therefore, will work in collaboration with nursing, EMS, respiratory therapy, laboratory, radiology, medical records, management, and administration in a professional manner and contribute positively to the GMHC strategic plan.
The APP performs the functions of examining, assessing, planning, and implementing the health care of patients within their scope of practice in conjunction with a collaborating physician via telehealth platform.
The APP secures patient histories, performs physical examinations, orders laboratory tests, develops and implements treatment plans, performs diagnostic and treatment procedures, records findings, and provides patient education and counseling to enhance our effectiveness in providing primary care for inpatient admissions.
Essential Duties:
Develops patient care plan.
Collaboratives with the telehealth physician or medical director as needed.
Uphold the organization's legal responsibilities related to HIPAA and EMTALA.
Facilitates referrals to community health services, agencies and resources and works collaboratively with case manager/discharge planners.
Recognizes medical emergencies and initiates evaluative and treatment procedures necessary in life-threatening situations.
Conducts patient rounds, recording pertinent information in the patient's medical record, accurately writing, or executing orders in collaboration with the supervising physician as needed.
Prepares transfer orders, discharge summaries, referral physician communication, home prescriptions, etc.
Consults with physicians, nursing staff, and technicians in order to evaluate standards of patient care provided and makes recommendations for improvement.
Participates in daily interdisciplinary team meeting Assists in quality improvement efforts by participating in quality improvement teams as requested.
Independently identifies program weaknesses/gaps in service and develops solutions to improving patient care and customer services.
Practices strong collaboration in planning for post-hospital care with the primary care providers and community agencies.
Consults with supervising physicians as medically appropriate.
Completes documentation in accordance with bylaws.
Demonstrates clinical excellence, models professional behaviors, maintains confidentiality and promotes a positive attitude amongst peers and staff.
Promotes patient rights and functions as a patient advocate.
Assists with the development of patient care policies, procedures and protocols.
Member of the Critical Access Hospital Committee Participates in the annual performance review process.
Maintains Basic Life Support (BLS) certification, Advanced Cardiac Life Support (ACLS) certification and Pediatric Advanced Life Support (PALS), Neonatal Resuscitation (NRP), and Advanced Trauma Life Support (ATLS).
Responds as appropriate to emergencies within the hospital, such as Rapid Response and Code Blue.
Assists in the selection and training/orientation of personnel in the hospital, as needed.
Assist with education of fellow provides and ancillary staff.
This includes appropriate discussion and knowledge transfer to patients, patient care staff, students, and medical staff.
Actively participate in departmental meetings, in-services, and other training sessions relative to the job.
Provides professional and community education regarding health issues as requested.
Participates in community service events as needed.
Annual emergency skills competency will be required through CME as directed Performs other duties as requested/assigned PRESCRIPTIVE AUTHORITY:
The NP prepares prescription orders for medications, including controlled substances, in accordance to all applicable state and federal laws.
